Absence Notification Form

Please use this form to notify the school if your child is absent due to illness or another reason, or to request permission for a planned absence during term time. Please provide full and accurate information so that we can update our attendance records and consider your request appropriately.

Please remember that submitting this form does not mean that a planned absence has been authorised. The school will contact you to confirm the outcome of any request.
